Pattern one: nobody owned the decision
Projects have moments when someone must decide quickly: the delivery arrived damaged, the slab reading came back high, the wall opened onto something unexpected. When ownership is unclear, the default is to keep working and decide later, which is the worst available option.
Fix it structurally. Name the decision maker for site conditions in the contract, and agree what triggers a stop. "Any moisture reading above the manufacturer's limit stops installation" is a rule that removes the argument in the moment.
Pattern two: a water event handled as an inconvenience
Rain through an open roof, a burst supply line, a failed temporary connection. These happen on live projects and the response window is short.

On a construction site the sequence is the same as in a finished home: stop the water, remove standing water, get air moving, dry the assembly, and measure rather than guess.
The material decision follows from the measurement. Insulation, gypsum board, engineered panels and packed cellulose that stayed wet generally have to go. A surface that looks fine after two days is not evidence that the cavity behind it is dry.

Pattern four: the schedule outranked the evidence
The most expensive projects share one feature: a piece of evidence arrived, it was inconvenient, and the schedule won. A high moisture reading, a suspect material, a damaged delivery, a failed test.
The counter-measure is boring and effective. Put the stop conditions in writing before work starts, agree who calls the stop, and agree who bears the delay cost in each case. Evidence that has a pre-agreed consequence gets acted on. Evidence that requires a fresh negotiation does not.
